The following form needs to be filled out for each candidate seeking the Sacrament of Confirmation. Once the form is submitted, you will receive a confirmation email with further instructions to submit additional documents.

5. Sponsor or Proxy

SPONSORS OR PROXY cannot be parents, spouses, or the baptismal godparents. Seminarians, priests, and religious will not be accepted as sponsors unless a signed letter of authorization from his/her superior is provided with the registration application. Sponsors must be at least 14 years of age, but preferably 16 or older, themselves confirmed, the same gender as the applicant, and a practicing Catholic who knows well his/her Faith. If the sponsor cannot physically be present at the ceremony, a proxy is needed to stand in for them.

REQUIRED: Sponsors who are not attending Mass at an SSPX FL church must provide a letter of good standing from their priest sent to St. Thomas More Priory, 500 Riverview Avenue, Sanford FL 32771, ATTN: Fr. Sean Gerrity.
